A 15-question screening to help Malaysian parents decide whether an occupational therapy assessment is worth pursuing for their child.

1. Does your child struggle to hold a pencil correctly by age 5?
2. Does your child have trouble using scissors, buttons, or zippers for their age?
3. Is your child clumsy, bumping into things or tripping more than other children?
4. Does your child avoid playground equipment like swings, slides, or climbing frames?
5. Is your child very sensitive to clothing tags, seams, or certain fabrics?
6. Does your child cover their ears at loud sounds (vacuum, hand dryers, parties)?
7. Is your child a very picky eater, refusing foods based on texture?
8. Does your child have trouble sitting still for meals or short activities?
9. Does the teacher report your child is inattentive or disorganised in class?
10. Does your child need help dressing or toileting beyond the typical age?
11. Does mealtime take much longer than it should, with frequent spills?
12. Does your child meltdown over small changes in routine?
13. Does your child struggle to play with other children of the same age?
14. Is your child's handwriting messy, slow, or causing hand pain?
15. Has a teacher, doctor, or nursery staff already raised a concern?
